Student Registration
Registration Requirements
The following documents are required to complete student registration:
- Child’s birth certificate, Social Security card, and current immunization records
- Parent/Guardian photo ID
- Notarized Act 26 Form (must be completed prior to admission)
- Proof of residency
(Current light, gas, water, or landline telephone bill. Rental leases are not accepted.) - Court documentation (for foster care or placement students)
- Final report card from the previous school
- Homeless students: completed required forms
If your child receives Special Education or Gifted Support services, please notify the registration office and provide a copy of your child’s IEP.
