Student Registration

Registration Requirements
 
The following documents are required to complete student registration:
  • Child’s Birth Certificate
  • Current Immunization Records
  • Proof of Residency (2 Current Utility Bills)
  • Parent/Guardian photo ID
  • Notarized Act 26 Form must be completed prior to admission (any NEW students to the district that attend a prior school in grades K-6)
  • Final report card from the previous school
  • Homeless Students: McKinney Vento forms are available
 
*Special Education/Gifted Support: If your child receives services, please provide a copy of their current IEP.
 
*Pre-K Counts Eligibility: Please provide proof of income (W-2, 3 pay stubs, or TANF).
